Application Process

Applications for Queen's School of Medicine are submitted to Ontario Medical Schools' Application Service (OMSAS). It is your responsibility to ensure that OMSAS receives all the required application pieces, including all references, documents, transcripts, and registrar statements, by the application deadline. You can find more information about OMSAS' key dates and application deadlines here.

All application components must be submitted to OMSAS by the specified deadlines for the application to be considered. Addendums or supplemental materials sent directly to the School of Medicine, or incomplete applications will not be considered. Applicants who have applied previously must resubmit all application requirements each cycle to be considered.

The Admissions Committee is unable to review and evaluate application documents that are not written in English. This will deem your application incomplete.

A lack of appropriate documentation by the application deadline will deem your application ineligible.

Is the Queen’s-Lakeridge Health MD Family Medicine Program for me?

This program is for applicants who can see themselves having a fulfilling career in family medicine and know they want to become family practitioners.

If you are unsure of what path you would like to pursue then the MDFM program is not for you. You may wish to consider the MD - Kington program instead.

This program has dedicated postgraduate Family Medicine Residency spots. These positions are set aside for successful graduates of this program without going through the CaRMS process under the authorization of the Queen’s Family Medicine Training Program. A purpose built transition to residency curriculum will prepare our graduates to enter their residency with advanced skills aiding in their ultimate transition to a practice in Comprehensive Family Medicine.

The program is based at the Queen’s satellite campus at Lakeridge Health in Durham Region.

If I apply to MDFM, will my application also be considered for the MD Program at the same time?

No. You must indicate your intention to apply to this program in your OMSAS application. By selecting this option, you are indicating that this is your first choice. You will only be considered for this program in the initial threshold review. If you select MDFM but do not submit all the required documents for the MDFM application, your file will be moved to the MD stream for the initial threshold review. Assessment of your application for the MD Kingston Campus Program will be identical to all other applications to this program and will in no way be influenced by the fact that you indicated interest in the MD Family Medicine Program.

You will not be considered simultaneously for both the MDFM and the MD Kingston Campus Program.

If you apply to the Queen’s-Lakeridge Health MD Family Medicine program you are NOT eligible to apply to the MMTP stream or MD/PhD, MD/Master’s combined program.

If I am invited to an MDFM interview, will I be considered for both MDFM and MD?

Your application will be moved to the MD Kingston Campus Program for consideration only if you are invited to an MDFM interview and are unsuccessful in receiving an offer for this program. Assessment of your application for the MD Kingston Campus Program will be identical to all other applications to this program and will in no way be influenced by the fact that you indicated interest in the MD Family Medicine Program.

You will not be considered simultaneously for both the MDFM and the MD Kingston Campus Program. If you are offered a position in the MDFM and decline, you will not be considered for the Kingston Campus MD program.

Does the Ordinarily Resident in Ontario Requirement apply to the MDFM Program?

Yes, the 95% Ontario residency admissions requirement, effective July 1, 2025, applies to both the MD Kingston Campus and the MD Lakeridge Campus. More information can be found on our Eligibility & Application Process Page.

Responses to a series of short answer questions in the application.

MDFM-Specific Confidential Assessment Forms

2 additional Confidential Assessment Forms (CAFs) in addition to the 3 CAFs that are required for the MD application (total of 5 Confidential Assessment Forms).
You may use the same referees for the Queen’s-Lakeridge Health MD Family Medicine Program and the mainstream MD CAFs; however, there are different questions in each form that will need to be completed. You will see the referee selection choices in the OMSAS Application.

All components of the main MD application and the supplemental items for the Queen’s-Lakeridge Health MD Family Medicine Program must be submitted by the deadline to be considered.
