Primary Sensory Modalities include:

Pain sensation (pinprick)

  • Can be tested using a pin, toothpick or broken tongue depressor
  • Focus your examination in the location of the sensory symptoms
  • If patient has burning pain in both feet, it is likely due to a polyneuropathy- you can start at the toes and work your way up the shin to determine the level where there is normal pain sensation (it should be in a stocking distribution). Typically you should not see abnormalities in the hands with a polyneuropathy until the abnormality in the legs reaches the knees.
  • However, people with polyneuropathy are more prone to also getting compressive neuropathies such as ulnar neuropathies or carpal tunnel syndrome.
