As a team, review the resources that you have accessed as part of this module, as well as the case study of Doug. Assume that you are Doug's Occupational Therapist, and he is now being discharged from the rehabilitation hospital into the community. Write a discharge summary specific to occupational therapy, including recommendations for Doug's integration back into the community. Begin with a brief summary of Doug's injury, and an overview of his demographic information. If there is anything missing in the case study, enter fictional information.

Your report should take into account goals and recommendations for seating and positioning, community mobility, transfers, toileting, feeding, vocational rehabilitation, psychosocial dimensions, independent living skills (ie: cooking, ADLs), environmental accessibility, and caregiver support. If there are any other applicable areas, feel free to include them.

When writing your report, be specific. For example, although Doug may already leave the hospital with a wheelchair, it is important for you to review exactly what type of wheelchair he has, and what features have been prescribed (eg. Doug has been prescribed a manual wheelchair with custom seating that includes lateral support bilaterally, a Roho cushion, and rigid backrest. His wheelchair has been adjusted with camber and squeeze to facilitate support and ease of mobility). Arrange the report in a way that makes sense to your team, and communicates your ideas well.

Your final report should not exceed 4-5 single spaced pages.
