Champlain has fostered an interconnected, well-supported regional mental health and addictions system—the first of its kind in Ontario! There are practice opportunities in rural and urban settings as well as community, acute and tertiary hospitals (many of which are associated with The University of Ottawa). The GBACHC delivers primary care, mental health services, nutritional counselling, rehabilitation therapies and community programs to fulfill our mission to empower health and wellbeing of the people in our community – together. Our interdisciplinary team includes: nurse practitioners, registered nurses, dietitians, physiotherapists, an occupational therapist, respiratory therapist, health promoters and an administrative team. Following the Community Health Centre Model of Health & Wellbeing, the GBACHC ensures all practitioners have time to support the whole person and make a difference to the most vulnerable people. Looking for balance, our community is situated along the beautiful west shore of Lake Huron and is within a 50-minute driving distance of London, ON and Stratford, ON. The area is known for its lake-view sunsets, hiking, bicycle trails, family farms and friendly people. Residents are served by tertiary health services at London Health Sciences Centre, Bluewater Health in Sarnia and a network of social service agencies and community hospitals, including: the Huron Perth Healthcare Alliance, South Huron Hospital and the Middlesex Hospital Alliance.
The Physician provides primary health care to clients in accordance with the Health Disciplines Act of Ontario, the practice standards of the College of Physicians and Surgeons of Ontario (CPSO) and Community Health Centre guidelines. The Physician practices evidence-based medicine and utilizes a holistic approach to client care, including acute curative care, promotion of healthy lifestyles and prevention of illness by involving the client in their diagnosis and treatment, and through sensitivity to non-medical problems and appropriate referrals both inside and outside the Centre. The Physician participates in case conferences and information sharing with the other members of the multidisciplinary clinical team. Through a major redevelopment project, the Hawkesbury & District General Hospital (HGH) is transforming into a full service regional hospital.
HGH is a bilingual community hospital in Prescott-Russell located 50 minutes between Montreal and Ottawa offering medical, obstetrical, surgical, emergency, rehabilitation, ambulatory and a broad range of diagnostic services. Our redevelopment project will transform HGH to a full-service regional 100-bed hospital with state-of-the-art medical equipment.
