Centre.

PATIENT INFORMATION COLLECTOR

An information
collector has been designed and is intended to help you
structure your cancer care information. The binder is
divided into sections for you to put in your personal
health and treatment information, sort out other information
you receive or collect and direct you to other sources
of information and professional resources. |
It is hoped that the binder
will assist you to:
- Communicate effectively
with your cancer care team
- Participate more fully
in your care
- Keep track of information
about your illness
- Keep valuable information
in one place and close at hand
This binder is a tool. Change
it to meet your needs. You can add sections, take sections
out and copy forms. Extra copies of pages are available at
the Cancer Information Kiosk in the main lobby or from our
web site www.krcc.on.ca. It is expected that this binder will
grow as you keep track of your personal journey. Please ask
your nurse or physician if one is not offered at your first
visit.
We have enclosed some general
information that all cancer patients need when they first
come to the Cancer Centre. Everyone has different needs and
paths along their journey with cancer, so different information
can be added as you require it. This binder is for you to
use in a way that best fits your needs but we do suggest that
you bring it along to your appointments at the Centre.
It is our hope that this Collector
will be helpful to you. If you do not think you will use it,
please return it so that others may benefit from it.

Reliable information
Information about all cancers, treatment, early detection
and resources available in your community.
Emotional support
One-to-one and group support from others who have had a similar
experience to yours. To be connected to a cancer survivor
or caregiver, call 1 800 263-6750.
Transportation
If you need help getting to your cancer-related care, the
Canadian Cancer Society can work with you to find the best
way to get you there.
Support at cancer centres
and lodges
Practical support for people at treatment centres and lodges.
For information about cancer or to connect with our support
services, please call 1 888 939-3333 toll-free or visit the
Canadian Cancer Society's website at www.cancer.ca.
Information is available in English and French, 24 hours a
day.
The site also helps visitors
locate support services in communities across Ontario through
the Community Services Directory. The directory can help users
find support services close to home. It can be searched 24
hours a day by a specific service name and/or location. It
lists approximately 4,000 community services, treatment locations,
wigs and prostheses.

TAX
RETURN EXPENSE LOG for 2011
Print this expense
log to help you keep track of meal and vehicle expenses
for income tax purposes. If you are unable to print this,
please ask for one at the Cancer Centre.

CANCER
INFORMATION KIOSK
Staff and volunteers of the
Cancer Centre are happy to help you obtain information about
your diagnosis or treatment. Please ask staff members for
information.
Please visit our Cancer Information
Kiosk located in the Main Lobby of the Centre. Volunteers
staff the Information Kiosk and are happy to help you select
materials that you need. The Information Kiosk has books,
pamphlets, audio-cassettes, CD ROMs and videos on cancer-related
topics. You may borrow any of the information to take home
or you may look at it while in the Centre. There are also
many free pamphlets that you can take with you.

ABOUT LUMBAR PUNCTURE
What is a lumbar puncture
(LP)?
A lumbar puncture is the insertion
of a needle into the fluid within the spinal canal. It is
termed a "lumbar puncture" because the needle goes
into the lower part of the back to obtain a sample of the
fluid in the spinal canal (the cerebrospinal fluid) for examination.
Another name for a Lumbar
Puncture is a spinal tap.
