In your orientation package, you will find a variety of information including your Letter of Appointment, an Application for Appointment to the House Staff, and information from KGH Occupational Health, Safety & Wellness. STEP 1: QUEEN’S POSTGRADUATE REGISTRATION
You are required to register online at https://meds.queensu.ca/medicine/postgrad/registration and you will need to satisfy the following criteria to complete the registration process. Please note that the online registration system will open on May 28, 2012.
Letter of Appointment
You will receive three copies of your Letter of Appointment. Two copies must be signed and returned within thirty days to prevent a delay in the College of Physicians and Surgeons of Ontario (CPSO) issuing your Certificate of Registration for Postgraduate Education.
Criminal Record Check and Vulnerable Sector Search
You must contact your local police station and complete the application for a Criminal Record Check (also referred to as a Criminal Background Check or Police Reference Check) including the Vulnerable Sector Search (VSS). This may normally be done on-line through the website for your local police force. Payment for this report is your responsibility and when it is ready, you must collect from your police station in person with proper identification. The report must be conducted within the last six months and must be submitted to the Postgraduate Medical Education Office before you will be allowed to register. It is a condition of employment that all new KGH staff, including residents, must consent to disclosure of criminal records and information including pardoned sexual offences. For Kingston residents, the website of the Kingston Police Force is www.police.kingston.on.ca
You are strongly advised to begin this process immediately upon receipt of your information package. If you cannot provide identification as noted, the only way your background check can be processed is by submitting fingerprints to the RCMP. The RCMP currently estimates that the delay in processing exceeds 120 days.
College of Physicians and Surgeons of Ontario (CPSO)
The CPSO is the licensing body for Ontario and it is your responsibility to ensure that you have received your Postgraduate Education certificate. If you do not have an application package for your Certificate of Registration for Postgraduate Education, you should contact the CPSO immediately. If you do not receive the certificate before the starting date of your appointment, you must contact the CPSO Registration Department to confirm that you are registered. You cannot begin your program without a CPSO Postgraduate Education Certificate.
Canadian Medical Protective Association (CMPA)
Residents are required to maintain malpractice coverage through the CMPA or an acceptable alternative. This is mandatory for all residents and if you do not have a CMPA application package, you may obtain one from the CMPA website. You cannot begin your program without CMPA malpractice coverage.
Queen’s Email, NetID, and Student Number
You must obtain a Queen’s University Network Identity (NetID) and Queen’s email account before you can begin the on-line registration process. Your Queen’s email account is important as all email correspondence from Queen’s University will be sent to this address only. Please note that after activating your NetID, it may take 24-48 hours before you can successfully log into the registration system. To obtain your Net ID and Queen’s email address (and learn how to forward messages to another email address), please visit the Queen’s NetID website at: http://www.queensu.ca/its/netid.html Your Student Number appears on your Letter of Appointment.
Advanced Cardiac Life Support (ACLS) CertificationPlease review the ACLS webpage for further information.
Registration Process
You are required to register on-line at http://meds.queensu.ca/postgraduate/registration once you have your CPSO certificate and CMPA coverage. A $550 provincial postgraduate registration fee is required to complete the registration process. You will be sent an email confirmation of your payment and registration once you have completed the process, but you may also wish to print a copy of your confirmation receipt as the Postgraduate Medical Education Office is not able to issue receipts. You should also contact the CPSO or CMPA if you have not received notification of the status of your certificate or malpractice coverage.
Address Change
If your address changes after you have registered on-line and obtained your student number and NetID, you will need to update your personal information with Postgraduate Medical Education, KGH People Services and Organizational Effectiveness, and the Office of the University Registrar.
Student Photo ID Cards
You will need to complete and return the enclosed information sheet to obtain a Queen’s student card. It should be returned to the Office of the University Registrar (OUR), located in Gordon Hall. Your student card may be collected after July 1 from the OUR. For Family Medicine residents in the Belleville-Quinte, Peterborough-Kawartha, and Bowmanville-Oshawa-Lakeridge programs, your student cards will be forwarded to your Program Assistants to distribute to you.
Orientation
A variety of orientation sessions have been organized by Postgraduate Medical Education and Kingston General Hospital. Details are provided on the Orientation website.
Paymaster Hospital
Kingston General Hospital is the paymaster hospital for all residents registered in a postgraduate residency program at Queen’s University. Paystubs will be distributed electronically to KGH email accounts. Any questions regarding salary or benefits should be directed to KGH People Services and Organizational Effectiveness at residents@kgh.kari.net.
Your registration must be completed by June 25 in order for you to begin your program on July 1. A late fee of $50 will be applied to all registrations after June 25. Please note that the CPSO, CMPA, and Queen’s will be closed on July 1 & 2. The PGME Office will notify the following people of residents who are not able to work as of July 1:
Please note that you will not be allowed to continue in your program if you are not completely registered.
STEP 2: KGH/HDH APPLICATION FOR APPOINTMENT TO THE HOUSE STAFF
Please forward completed forms to the People Services and Organizational Effectiveness Office, Watkins 4, KGH as soon as possible. Your application should be made well in advance of the expected date of appointment as the credentialing process is lengthy.
The Appointment procedure has the following steps:
STEP 3: OCCUPATIONAL HEALTH, SAFETY, & WELLNESS DOCUMENTATION
As an employee of Kingston General Hospital, there is specific information you need to be aware of prior to starting or continuing in your residency program. You are required to read the enclosed Workplace Health and Safety Review and acknowledge having done so by signing the enclosed form. You are also requested to complete the Release of Health Information portion of this form so that OHSW may release your employee health information to any hospital to which you may be assigned during the course of your training. This form must be returned to KGH Occupational Health, Safety, & Wellness (OHS&W).
The OHS&W Department is open Monday-Friday (0700-1600) and Occupational Health Nurses (OHNs) are on call weekends and holidays (0700-1900) to deal with more urgent matters and may be reached by calling the switchboard. In addition, an Occupational Health Physician is available for consultation upon referral by the OHN.
The OHS&W department provides the following services:
