Registration - Continuing Residents

 

We are pleased to welcome you back to the next year of your postgraduate program.  In this registration package, you will find a variety of information including your Letter of Appointment, an Application for Reappointment to the House Staff, and information from KGH Occupational Health, Safety & Wellness.  If you have any questions after reviewing the enclosed material, please do not hesitate to contact our office at pgme@queensu.ca or by telephone at (613) 533-2543.

 

STEP 1:            QUEEN’S POSTGRADUATE REGISTRATION

You are required to register online at https://meds.queensu.ca/medicine/postgrad/registration and you will need to satisfy the following criteria to complete the registration process.  Please note that the online registration system will open on May 28, 2012.

 

Queen’s NetID and Student Number

You will need your Queen’s NetID and Student Number.  If you don’t know your NetID or have forgotten it, please consult the FAQ section of the NetID website at www.queensu.ca/its/netid.html   Your Student Number appears on your Letter of Appointment.

 

Queen’s Email Address

The use of a Queen’s email address is required as all correspondence from the Postgraduate Medical Education (PGME) Office and the University Registrar will be sent only to your Queen’s address.  To find out how to forward your Queen’s email to another account, please consult the email FAQs at www.queensu.ca/its/email/faqs.html

 

Letter of Appointment

You must sign and return two copies of your Letter of Appointment to the PGME Office within thirty days.

 

CPSO License Renewal

Your College of Physicians and Surgeons of Ontario (CPSO) license must be renewed and will be if you have returned two signed copies of your Letter of Appointment to the Postgraduate Medical Education Office AND paid the license renewal fee to the CPSO.  If you have not received your renewal certificate before the starting date of your appointment, you must contact the CPSO Registration Department to confirm that you are registered.

 

CMPA Membership

Your Canadian Medical Protective Association (CMPA) membership must be renewed and will be if you have paid the renewal fee OR renewed the pre-authorized payment plan.  If you have not received confirmation of renewal by the end of May, please contact the CMPA.



On-line Registration

Once all of the above criteria have been met, you may register on-line and an email will be sent to you before June 1 with registration instructions.  Please note that you will also be required to pay the provincial postgraduate registration fee of $550 at the time of registration by credit card.  You are advised to print your confirmation receipt at the time of payment as receipts are not issued by the PGME Office.

 

Your registration must be completed by June 25 in order for you to begin your program on July 1.  A late fee of $50 will be applied to all registrations after June 25.  Please note that the CPSO, CMPA, and Queen’s will be closed on July 1 and 2.   The PGME Office will notify the following people of residents who are not able to work as of July 1:

  • Director, Medical Affairs, Kingston General Hospital
  • Associate Dean, Postgraduate Medical Education
  • Your Program Director
  • Financial Services - Payroll, Kingston General Hospital
  • University Registrar

 

Please note that you will not be allowed to continue in your program if you are not completely registered.


 

STEP 2:            KGH/HDH APPLICATION FOR REAPPOINTMENT TO THE HOUSE STAFF

 

Please forward completed forms to the People Services and Organizational Effectiveness (PSOE) Office, Watkins 4, KGH as soon as possible.  The credentialling process is lengthy, so applications should be submitted well in advance of the expected date of appointment.

The Appointment procedure has the following steps:

  1. Complete the enclosed Application for Reappointment to the House Staff.  For Family Medicine Residents in the Belleville-Quinte, Peterborough-Kawartha, and Bowmanville Oshawa-Lakeridge Programs, please complete the appropriate applications for housestaff privileges at your respective hospitals.
  2. The Postgraduate Medical Education Office will confirm your registration in the School of Medicine, Faculty of Health Sciences, Queen’s University, CMPA Membership, CPSO Educational Licence and your agreement concerning the Collection and Use of Personal Information.
  3. When complete, your application will be submitted to the PSOE Office.  When all required documentation is received, your name will be included on a listing indicating completion of all hospital registration requirements which will  be forwarded to the Chief Executive Officer or delegate who shall immediately refer the listing to the Combined MAC Credentials Subcommittee.  It will then be forwarded to the Medical Advisory Committee which in turn will make recommendations to the Board of each hospital, which makes the final decision.
  4. Should you be refused appointment, you may apply for a hearing under Sections 37 and 39 of the Public Hospitals Act of Ontario.
  5. You must sign and date the enclosed Resident Contract Letter. This must be returned to the PSOE Office as soon as possible either in person, by fax, or by scanning and emailing a copy to residents@kgh.kari.net.  When the contract is received at PSOE, it will be signed by a hospital representative.

 

STEP 3: OCCUPATIONAL HEALTH, SAFETY, & WELLNESS DOCUMENTATION

The OHS&W Department is open Monday-Friday (0700-1600) and Occupational Health Nurses (OHNs) are on call weekends and holidays (0700-1900) to deal with more urgent matters and may be reached by calling the switchboard.  In addition, an Occupational Health Physician is available for consultation upon referral by the OHN.

The OHS&W department provides the following services:

  • Communicable disease surveillance- screening and immunizations 
  • Post-exposure follow up & testing
  • First Aid
  • Illness management including approval of short term disability leaves of absence and disability case management
  • Injury management and reporting to the Workplace Safety & Insurance Board (WSIB) and Ministry of Labour as required 
  • Respirator (N95) Fit testing & training
  • Development and implementation of health & safety initiatives/programs to ensure compliance with the Occupational Health & Safety Act and to improve organizational health & safety 
  • Serve as resources to employees on health & safety issues/concerns and make recommendations to address/resolve health & safety issues.
  • Health Promotion/wellness activities


You must review and ensure compliance as required with the following information:


1. Requirements for Continuing Tuberculosis Surveillance
For those Residents who perform activities at KGH deemed to be "high risk" (cough-inducing procedures, bronchoscopy, respiratory therapy, autopsy, morbid anatomy and pathology exams, or manipulation of mycobacterium cultures), an annual TB skin test is required. For those residents who will be completing a rotation in another healthcare facility this year, a repeat TB skin test is required to your placement at that facility. TB skin testing can be done through KGH Occupational Health Service, Watkins 1.

2.Respirator (N95) Fit Testing and Training
It is a mandatory requirement that all residents have a valid N95 Respirator. Respirators are only valid for a period of 2 years. You must ensure that your N95 Respirator test is current and the information is recorded by Occupational Health and Safety at Kingston General Hospital. Please forward the mask fit test information to residents@kgh.kari.net.

3. Additional Immunizations
Three additional immunizations are required by residents completing rotations at the University of Ottawa. Please ensure that you have completed, and updated your immunization record,  with the following: a polio series, d-Tap series, and Td series. You may contact OHS&W to update your immunization record or obtain information on completing the required immunizations.
