Conference Funding forms:

Conference Funds Request Form (IM program) - complete before conference
Reimbursement Form Request (Queen's form)  - This form is for expenses reimbursement after March 31, 2012 and must be submitted to program office 30 days after returning. 

Elective Request form:

This form is required so that we know where you are during your elective period, who your supervisor is, and what your goals & objectives are for your elective. Please submit this at least 2 blocks before elective start date.

Evaluation forms:

Please use the specific ITER for your chosen elective.
Mini-CEX Form - use this for Portfolios (expected at least 1 per rotation)

Handover form:

Medicine Admissions Handover - for use on call
